check with your insurance company. If you have difficulty

help you with this.

|
| What can I expect when I arrive for my first appointment? |
Please arrive 15 minutes before your appointment so that all
paperwork can be completed. You will then meet with
a physical therapist. Your therapist will take a medical
history, do an exam and then discuss the findings with you. You
and your therapist will work together to develop a treatment plan
and goals for recovery. Treatment will begin at your first
appointment. You will probably be instructed in exercises
to do at home. Doing your exercises every day will speed
your recovery. You should allow 1 to 11/2 hours for your
first appointment.

| Should I wear special clothing? |

|
|

Please bring your insurance card, your referral (if you have
one or your insurance requires it) and a list of your medications. Please
complete the patient intake form and bring that with you. LINK

|
| Will my physical therapist communicate with your doctor? |
Yes. A copy of your initial evaluation, periodic updates
and a discharge evaluation are sent to your doctor. Please
let us know when you have an appointment with your doctor. We
will send an update to your doctor prior to your appointment.

|
| How will I benefit from physical therapy? |
Our goal at O’Neill Physical Therapy is to “Get you
Back in Action”. You and your therapist will discuss
your goals. Generally these include decreasing or eliminating
your pain, returning to your normal activities and learning ways
to prevent future problems.

your copays and deductible. If you have trouble getting this
information you may call our insurance office at 301-733-8060 and
we will call your insurance company.
